The endogenous cannabinoid system is a ubiquitous lipid signalling system that is evolutionarily ancient and plays an important regulatory role in all vertebrates.Cannabinoid endocannabinoids, cannabis-like substances, are small molecules formed from arachidonic acid, i.e. ANANDAMIDE, and 2-ARAHYHAHIDONOYLGLYCEROL. They bind to group G protein receptors, of which CANNABINOID-CB1 receptors are densely distributed in many brain regions associated with motor control, cognition, emotion, motivation and homeostasis.
Outside the brain, the EC system is a very important modulator of the autonomic nervous system, immunity and microcirculation. Endocannabinoids are released from lipid precursors in a receptor-dependent manner and serve as carriers of retrograde signalling at gabaergic and glutaminergic synapses. They also act as modulators of postsynaptic transmission by interacting with other neurotransmitters such as dopamine.
Endocannabinoids enter the cell through a specific system and are degraded by two known enzymes: amide hydrolase and monoacyl glycerol lipase.
Modern pharmacology has synthesised cannabinoid receptor agonists and antagonists: these are anandamide uptake inhibitors and selective inhibitors of endocannabinoid degradation.
These preparations are used to treat pain, obesity, anxiety, addiction, alcoholism, etc. For alcoholism, cabinoid receptor antagonists are used.
The logic of medical cannabis treatment is its binding to endocannabinoid receptors and the stimulation of biological processes related to this system.
Its use is wide-ranging, in neurology and internal medicine as well as ophthalmology. It gives new hope to patients.
